Health Assessments
Health assessments are preventative screening tools used to identify early indications of illness, disease or health risks. The screenings include not only physical & biomedical assessments but also emotional, social and environmental. Our main goal is to see you lead a healthy, independent, productive, and enjoyable quality of life. Our assessments are performed by our Practice Nurse and your regular GP to promote holistic well-being and lifestyle approach, and all are fully covered under the Medicare Benefits Scheme. Any identified risks will be addressed, and private referrals or follow-up care will be organised as indicated.

75+ Health Assessment
As we age our physical and cognitive health decline gradually, anyone over the age of 75 years is therefore eligible for an annual health assessment.
This assessment includes:
Full blood check-up
ECG
Full set of observation (height/weight/blood pressure/blood sugar level/urinalysis)
Mental health screening
Nutritional and physical activity review
Continence assessment
Mobility and independence assessment
Comprehensive family and health history review
Vaccination status
Medication review
Identification and setting up of domestic, nursing or maintenance support in your own home
This assessment can take between 40 and 60 minutes to complete and covers services such as setting up an Advanced Health Directive, Power of Attorney and MyAgedCare,
We also offer completing the annual Drivers Medical Assessment in the same appointment, to save you time and money.
If we find any health issues, we will arrange for further appointments with your doctor for thorough investigation.

45-49 Health Assessment
This health assessment is a once-off assessment provided to those between the ages of 45-49 that are considered at risk of developing a chronic illness. This assessment will take between 30-60 minutes to complete and is aimed at identifying lifestyle risk factors that may eventuate or contribute to a chronic illness. Steps will be taken then to limit or reverse the risk and improve the quality of life for you and your family.
This assessment includes:
Full blood check up
ECG
Full set of observation (height/weight/blood pressure/blood sugar level/urinalysis)
Mental health screening
Nutritional and physical activity education
Lifestyle coaching
Comprehensive family and health history review

40-49 Health Assessment
This health assessment is available every 3 years to those between the ages of 40-49, that are considered at risk of developing Type II Diabetes. This assessment will take between 30-60 minutes to complete and is aimed at identifying lifestyle risk factors that may eventuate or contribute towards diabetes. Steps will be taken then to limit or reverse the risk and improve the quality of life for you and your family.

ECG
Full set of observation (height/weight/blood pressure/blood sugar level/urinalysis)
Mental health screening
Nutritional and physical activity education
Lifestyle coaching
Australian Diabetes Risk Assessment tool
Comprehensive family and health history review

Aboriginal & Torres Strait Health Assessments
This health assessment is available every 9 months to those of Aboriginal and/or Torres Strait origin at any age. Aimed to assist in Closing the Gap in health and social well-being, this assessment can take between 30-60 minutes to perform. Any identified risks will be addressed, and referrals or follow-up care will be organised with respect for culture. This health assessment also provides an additional benefit of 5 discounted referrals to certain allied health services per calendar year.

Family and social wellbeing and inclusiveness

Heart Health Check
This health assessment is available every 12 months to those from the age of 30 years that have or are considered at risk of developing a heart-related disease, aimed at identifying and reducing the risk of heart related disease. This service cannot be claimed if another health assessment has been performed within the last 12 months (excluding Aboriginal Torres Strait Assessment). This assessment will take between 20-30 minutes to complete.

Cholesterol, blood sugar, kidney function blood tests
ECG
Full set of observation (height/weight/blood pressure/blood sugar level)
Lifestyle coaching and advice
Nutritional and physical activity education
Comprehensive family and health history review
Starting medications for high-risk individuals
