These plans aim to improve the overall health and quality of your life by offering personalized and ongoing support.

  • Individualised care: Care Plans are tailored to meet your unique needs and preferences and taking into account your health goals and create a personalized care strategy.

  • Team care approach: A team of healthcare professionals, including general practitioners, nurses, dietitians, physiotherapists, and other specialists, work together to provide coordinated care to ensure all aspects of patient care is addressed.

  • Regular reviews: Regular assessments and monitoring are essential to track the progress of your condition. These assessments help in identifying any changes or complications early and taking timely interventions. Regular reviews are done 3 monthly or 6 monthly depending your requirements and severity of your condition.

  • Treatment plan: Care Plans outline the appropriate treatment plans and follow ups for chronic conditions. This includes prescribing medications, monitoring their outcome, side effects and seeking specialists’ advice. 

  • Health educations: During the care plans sessions your doctor and nurse will provide you adequate education on lifestyle measures such as healthy diet, exercise, weight management and prevention of alcohol/ smoking/ drug misuse.

  • Preventive care: During care plan your nurse will advise and take actions on preventive care including regular screenings, vaccinations, and promoting healthy habits.
